94 Notifying director-general about change in eligibility for authorisation An authorised practitioner must give the director-general written notice about any of the following events or circumstances within 14 days after the day the practitioner becomes aware of the event or circumstance: (a) a change to the authorised practitioner’s name; (b) a change to the authorised practitioner’s contact details; (c) a change to the authorised practitioner’s eligibility to be an authorised practitioner; (d) any other event or circumstance prescribed by regulation.
