15 Recording first request in individual’s health record If an individual makes a first request, the health practitioner must record the following information in the individual’s health record: (a) that the first request was made; (b) the health practitioner’s decision to accept or refuse to accept the first request; (c) if the health practitioner refused to accept the first request—the steps taken by the health practitioner to comply with section 14 (3) (b).
