13 Making first request (1) An individual may make a request for access to voluntary assisted dying to a health practitioner (a first request). (2) The request must be— (a) clear and unambiguous; and (b) made personally by the individual. (3) The request may be made in writing or orally, or by communicating in any other way the individual can.
