126 Duplicate licence (1) If, on the application of a licensee, the Director is satisfied that a licence or temporary licence has been destroyed, lost or stolen, the Director may issue a duplicate licence to the licensee. (2) An application for a duplicate licence is to be: (a) in a form approved by the Director; and (b) accompanied by the prescribed fee.
