38 Application for review (1) A person who is aggrieved by a decision of a public sector (a) under Division 2 (Accessing government information) on an application for access to government information, including a decision to charge a fee for providing access to the (b) under Division 3 (Correcting personal information) on an application to correct personal information; may apply to the public sector organisation for a review of the decision. (2) An application for review under subsection (1) is to: (c) identify the decision and the application on which it was made; and (d) set out the reasons the applicant is seeking a review of the decision; and (e) specify an address to which correspondence regarding the review may be sent to the applicant.
