#### 22 Application form—application for approval of health service (1) For the purposes of subsection 41(1) of the Act, the prescribed form, in accordance with which an application under that subsection by an organization for approval of a health service shall be made, is Form 2 in Schedule 1. (2) An application under subsection 41(1) of the Act by an organization shall be signed by a person, being one of the persons responsible for the management of the organization, authorized in writing by the organization to sign the application.
