#### 23DZP Application procedure (1) An application for registration of diagnostic imaging premises or a base for mobile diagnostic imaging equipment must: (a) be in writing; and (b) be in accordance with the approved form; and (c) include the primary information; and (d) include the other information prescribed for the purposes of this paragraph. (2) Information may only be prescribed if it is relevant to the purposes for which the Register is kept.
