30  Application for the FWC to deal with a dispute about an extension of a period of unpaid parental leave  (1) A referral to the FWC under subsection 76B(3) of the Act, of a dispute between an employer and an employee relating to a request under subsection 76(1) of the Act for an extension of unpaid parental leave, must be made by application to the FWC.  (a) a copy of the written request; and  (b) if a written response has been given under subsection 76A(1) of the Act at the time of lodging the application—a copy of the written response.
