sec.44 Form of application
An application for a service approval must— be made to the Regulatory Authority of the participating jurisdiction in which the service is to be located; and be in writing; and include the prescribed information; and nominate one or more individuals to be nominated supervisors of the service; and include from each nominated individual (other than the approved provider) the written consent to the nomination; and include payment of the prescribed fee. An application for service approval may include an associated children’s service. An application for a service approval for a family day care service may include a request for approval of a place (other than a residence) as a family day care venue for that service. This approval is granted under section 50A only if exceptional circumstances exist. s 44 amd 2017 No. 9 (Vic) s 11 (sec.44-ssec.1) An application for a service approval must— be made to the Regulatory Authority of the participating jurisdiction in which the service is to be located; and be in writing; and include the prescribed information; and nominate one or more individuals to be nominated supervisors of the service; and include from each nominated individual (other than the approved provider) the written consent to the nomination; and include payment of the prescribed fee. (sec.44-ssec.2) An application for service approval may include an associated children’s service. - (a) be made to the Regulatory Authority of the participating jurisdiction in which the service is to be located; and
- (b) be in writing; and
- (c) include the prescribed information; and
- (d) nominate one or more individuals to be nominated supervisors of the service; and
- (da) include from each nominated individual (other than the approved provider) the written consent to the nomination; and
- (e) include payment of the prescribed fee.
