sec.40 Requirements for application

An application for internal review of an original decision must— be in the approved form; and if the affected person has been given an information notice for the decision—include enough information to enable MEDQ to decide the application; and be made to MEDQ within— if the affected person has been given an information notice for the decision—20 business days after the day the person is given the notice; or if the affected person has not been given an information notice for the decision—20 business days after the day the person becomes aware of the decision. MEDQ may, at any time, extend the period within which the application may be made. Subject to section 42 , the application does not affect the operation of the original decision or prevent the decision being implemented.
