sec.77 Requirements for certificates
A competent person certificate must— be in the approved form; and be signed by the competent person giving the certificate; and state— the basis for giving the certificate; and the extent to which the competent person has relied on tests, specifications, rules, standards, codes of practice or other publications.
- (a) be in the approved form; and
