What it does
This instrument is an Airworthiness Directive (AD) directed at Sikorsky S-92 Series helicopters, designated AD/S-92/5 (Tail Rotor Pitch Change Bearing Assembly), issued by the Civil Aviation Safety Authority and signed by Charles Lenarcic as Delegate of CASA on 27 August 2007. The AD became effective on 30 August 2007. It identifies a specific part-numbered tail rotor pitch change shaft and bearing assembly (part number 92358-06303-041) installed on Model S-92A helicopters as the subject of mandatory inspection and remedial action.
Mechanically, the AD requires borescope inspections of the affected shaft and bearing assembly in two distinct access locations and prescribes replacement where the part fails inspection. The inspection steps are not described in full within the AD text itself; rather the AD directs operators to follow specified paragraphs and figures of Sikorsky Aircraft Corporation Alert Service Bulletin No. 92-64-002, dated 3 August 2007 (ASB No. 92-64-002). The AD also references Maintenance Manual SA S92A-ANM-000 as pertaining to this subject. The AD states three discrete compliance timeframes: (1) a primary inspection window within 20 hours time-in-service (TIS) after the effective date of the AD; (2) an inspection window between 10 and 15 hours TIS after installing a shaft and bearing assembly; and (3) an instruction that appears in the Compliance cell as “Before further flight after the effective date of this AD.” The AD also notes that FAA AD 2007-17-05 Amendment 39-15163 dated 9 August 2007 is referred to.
The AD is prompted, in the document’s Background paragraph, by an incident involving failure of a shaft and bearing assembly and a servo clevis shaft that resulted in loss of tail rotor control. The stated safety purpose in the AD is to prevent failure of the shaft and bearing assembly, subsequent loss of tail rotor pitch and yaw control, and loss of control of a helicopter. The operative requirements are set out in the AD’s Requirement box: (1) borescope inspect each affected shaft and bearing assembly at the tail rotor side following the ASB Accomplishment Instructions paragraphs 3.A.(1) through (7) and Figure 4, and inspect each assembly on the servo side through the oil filler cap following ASB paragraphs B.(1) through (9) and Figures 2 and 3; (2) carry out the inspections detailed in Requirement 1; and (3) replace the shaft and bearing assembly if it fails the inspection. The AD contains two embedded notes: Note 1 pointing to Maintenance Manual SA S92A-ANM-000 and Note 2 pointing to the FAA AD mentioned above.
