What it does
This instrument is an Airworthiness Directive (AD) issued by the Civil Aviation Safety Authority (CASA) addressing safety concerns with Textron Lycoming piston engines and engine parts that were last overhauled or released by Provence Aéro Maintenance, France on or after 1 October 1997. The AD (AD/LYC/111, dated 4/2005 and effective 14 April 2005) requires owners/operators of affected engines to (a) submit specified documentary and operational information to the French Directorate General for Civil Aviation (DGAC) SFACT / N.ME by fax, and obtain from DGAC the "last date for conformity inspection and serviceability restoration", and (b) send the engine(s) and engine part(s) to a CASA-approved engine overhaul shop for a conformity inspection and restoration. The required submission is defined in the AD as a copy of the JAA Form 1, flight hours since the last overhaul, details of maintenance since the last overhaul, and details on significant maintenance actions and any findings of improperly performed work and steps taken for restoration (Requirement 1). The AD also provides a fallback if no response is received from DGAC within 30 days after providing that information, directing the submitter to contact CASA by fax or email and supply all the same information.
Compliance timing is mandatory: Requirement 1 must be complied with within 30 days after the AD’s effective date; Requirement 2 must be complied with either as per a date advised by the French DGAC or within 90 days after the AD’s effective date, whichever is later. The AD explicitly references French DGAC Airworthiness Directive F-2005-023 dated 2 February 2005, and records that the French DGAC cancelled the holder’s JAR 145 approval following an investigation into an engine failure attributable to multiple non-conformities. The AD identifies specific non-conformities discovered in the investigation, including an improperly assembled crankshaft counterweight bearing, non-conforming bolts, missing nuts, and improperly installed clips, and records that such non-conformities may affect safety of flight. The AD is signed by James Coyne, Delegate of CASA, dated 4 March 2005.
