sec.178 Particulars of claimant and worker
A notice of claim must include— the worker’s— full name and any other known names; and residential address; and date of birth; and gender; and usual occupation and, if that differs from the nature of employment at the time the worker was injured, the nature of the employment at that time; and the name and address of every employer of the worker at the time the worker was injured. Also, if the claimant is not the worker, the notice of claim must include the claimant’s— full name and any other known names; and residential address; and date of birth; and gender. (sec.178-ssec.1) A notice of claim must include— the worker’s— full name and any other known names; and residential address; and date of birth; and gender; and usual occupation and, if that differs from the nature of employment at the time the worker was injured, the nature of the employment at that time; and the name and address of every employer of the worker at the time the worker was injured.
