529N Information about persons conducting a business or undertaking

For section 529L(2), the person conducting a business or undertaking must give the following information about the person conducting a business or undertaking—

(a) the person’s name or, if the person conducts the business or undertaking under a business name, business name,

(b) the industry in which the business or undertaking is primarily conducted,

(c) Australian Business Number,

(d) phone number,

(e) email address,

(f) business address,

(g) the name of a contact person who is an individual,

(h) if the person must hold a policy of insurance under the Workers Compensation Act 1987—the number of the current policy of insurance.
