117 Notification of change of name, address or telephone number

The holder of an authorisation issued under section 28A of the Act or a permit issued under the Act must notify the relevant body of any change of name, address or telephone number provided to the relevant body in relation to the authorisation or permit within 10 business days of that change.

Penalty: In the case of a natural person, 5 penalty units;

In the case of a body corporate, 10 penalty units.
