#### 66 Patient to appoint contact person 66 Patient to appoint contact person > > (1) The patient must appoint a person as the patient’s contact person. > > > (2) A person is eligible for appointment if the person is an adult. > > > (3) Without limiting who may be appointed as the contact person, the patient may appoint— > > > > > (a) the patient’s coordinating practitioner, or > > > > > (b) the patient’s consulting practitioner, or > > > > > (c) another registered health practitioner. > > > (4) A person must not be appointed as the contact person unless the person consents to the appointment. > > > (5) The patient may revoke the appointment of the contact person. > > > (6) If the patient revokes the appointment of the contact person— > > > > > (a) the patient must inform the person of the revocation, and > > > > > (b) the person ceases to be the contact person for the patient on being informed under paragraph (a), and > > > > > (c) the patient must make another appointment under subsection (1).
