#### 61 Coordinating practitioner to notify Board about prescription of substance 61 Coordinating practitioner to notify Board about prescription of substance > > (1) Within 5 business days after prescribing a voluntary assisted dying substance for the patient, the patient’s coordinating practitioner must— > > > > > (a) complete the approved form for the prescription of the voluntary assisted dying substance (the prescription form) including the information required by subsection (2), and > > > > > (b) give the Board a copy of the prescription form. > > > > Maximum penalty—100 penalty units. > > > (2) The prescription form must include the following— > > > > > (a) the patient’s name, date of birth and contact details, > > > > > (b) the coordinating practitioner’s name and contact details, > > > > > (c) a statement confirming the coordinating practitioner has complied with section 73(2) or (3), > > > > > (d) the date the prescription for the voluntary assisted dying substance was issued, > > > > > (e) the coordinating practitioner’s name and the date the form was signed.
