90 Offence to fail to give copies of forms to the Board (1) A person who is required in accordance with this Act to give a copy of a form to the Board must give that copy to the Board in accordance with this Act. Penalty: 60 penalty units. (2) In this section, ***form*** means— (c) a final review form; or (d) a voluntary assisted dying substance dispensing form; or (e) a voluntary assisted dying substance disposal form; or (f) a co-ordinating medical practitioner administration form.
