64 Person may make administration request (1) A person may make an administration request, to the co-ordinating medical practitioner for the person, that the co-ordinating medical practitioner administer to the person the voluntary assisted dying substance specified in a practitioner administration permit, if— (a) the person is the subject of the permit; and (b) the person has decision-making capacity in relation to voluntary assisted dying; and (c) the person's request to access voluntary assisted dying is enduring; and (d) the person understands that the voluntary assisted dying substance is to be administered immediately after the making of the administration request. (2) The person must make the request personally. (3) The person may make the request verbally or by gesture or other means of communication available to the person. (4) An administration request must be made in the presence of a witness. (5) The co-ordinating medical practitioner must refuse to accept the person's administration request if the co-ordinating medical practitioner is not satisfied of any matter under subsection (1).
