63 Pharmacist at dispensing pharmacy to record and notify of disposal of returned voluntary assisted dying substance (1) A pharmacist at the dispensing pharmacy who disposes of a voluntary assisted dying substance under section 62 must immediately record that disposal in a voluntary assisted dying substance disposal form. (2) The pharmacist must within 7 days after disposing of the voluntary assisted dying substance give a copy of the completed voluntary assisted dying substance disposal form to the Board.
