41 Final review by co-ordinating medical practitioner on receipt of final request (1) On receipt of a final request made by a person, the co-ordinating medical practitioner for the person must— (a) review the following forms completed under this Part— (i) the first assessment report form; (ii) all consulting assessment report forms; (iii) the written declaration; (iv) the contact person appointment form; and (b) complete the final review form in respect of the person; and (c) certify whether the request and assessment process has been completed as required by this Act. (2) The co-ordinating medical practitioner must give the Board a copy of the completed final review form, accompanied by copies of all forms referred to in subsection (1)(a), within 7 days after completing the final review form.
