7 Particulars and documents required for registration

For the purposes of section 18(2) of the Act, an application for registration is to –

(a) contain the following prescribed particulars:

(i) the full name, honorary title and residential address of the applicant;

(ii) the proposed business address of the applicant, if known;

(iii) the speciality of the applicant, if any; and

(b) be accompanied by the following prescribed documents:

(i) a certified copy of the qualifications of the applicant;

(ii) proof of identity of the applicant.
