45KMaking a claim

#### 45K Making a claim (1) To be a proper claim, the claim must be: (a) made in writing; and (b) in accordance with a form approved by the Commission; and (c) accompanied by any evidence that the person making the claim considers may be relevant to the claim; and (d) lodged at an office of the Department in Australia in accordance with section 5T. (1A) A claim lodged in accordance with section 5T is taken to have been made on a day determined under that section. (2) The approved form may require the claimant to disclose whether the claimant is registered as a member of: (a) the pension bonus scheme (see Part IIIAB); or (b) the corresponding scheme under Part 2.2A of the Social Security Act.
