6 Parentage testing procedure reports
(1) A parentage testing procedure report must be prepared in accordance with Form 3.
(2) Part 1 of the report must be completed by the nominated reporter identified in the report.
(3) Part 2 of the report must be completed by—
(a) the person who carried out the parentage testing procedure, or
(b) the person under whose supervision the parentage testing procedure was carried out.
(4) A parentage testing procedure report completed otherwise than in accordance with this clause is taken to be of no effect.
