Strict temporal windows. Factor 1 requires that if smoking ceased, clinical onset must occur within ten years. A veteran who quit smoking at age 30 and developed RA at age 45 (15 years later) would not qualify, even if the smoking contributed. The same ten-year window applies for silica exposure (factor 2). The worsening window for smoking is only two years (factor 6), much tighter. Claimants must precisely date their clinical onset or worsening.

Smoking threshold is five pack-years. This is not trivial. A person who smoked 10 cigarettes per day for 10 years has exactly 5 pack-years. Smokers of light cigarettes, occasional smokers, or those who smoked only in group settings may fall short. The equivalent for pipe or cigar tobacco is based on weight; many smokers may not have records of how much tobacco they consumed.

Silica exposure requires 5,000 hours over ten years. That is over 1.5 hours per day, every day, for ten years. For someone who worked in construction or drilling for two years full-time (approximately 4,000 hours based on 40-hour weeks), they would need additional exposure to reach 5,000 hours, and it must be within the ten years before onset. The hours count only time spent during specific activities (production, excavation, drilling, cutting, grinding, or use in construction, manufacturing, cleaning, or blasting). Incidental exposure (e.g., being in the vicinity of silica dust) does not count.

The drug factor (3 and 8) is limited to two drugs (GM-CSF and interferon-α) and requires treatment within eight weeks before onset or worsening. This is a narrow factor. Many veterans with RA will not have had these drugs; they are not standard RA treatments. Obesity requires BMI ≥30 for at least five years within ten years before onset or worsening. BMI history is often not well documented. A claimant who was obese for four years and then lost weight before developing RA would not meet the factor. Even if obese at onset, the five-year duration must be proven. The instrument uses “being obese” as a continuous state, but the five years need not be continuous? It says “for at least five years within the ten years before the clinical onset”. Discontinuing DMARDs suddenly: factor 9. This requires continuous treatment for at least six months before cessation and worsening within six months. “Suddenly” is undefined. But if the drug was discontinued because it was ineffective and replaced with another, the worsening may be due to the RA progression itself, not the cessation. Inability to obtain appropriate clinical management (factor 12) is undefined. It may cover situations where a veteran was posted to a remote location without access to rheumatology, but the claimant must show that the inability caused the onset or worsening. This is a high bar. The decision-maker will consider what management would have been appropriate and whether the veteran could have accessed it. Cross-referencing trap. Factor 4 (periodontitis) or factor 3 (drugs) may require proof of the factor under another SoP. The claimant may need to satisfy two separate SoPs to make the RA claim. This multiplies compliance burdens.

Relationship to service. Even if a factor exists, it must be related to service. For example, smoking or obesity developed after service may not be related to service unless the service contributed to it (e.g., stress-induced smoking, weight gain due to service injury). The instrument does not define “related to” other than the generic requirement. This is a common area of dispute.

Definitions not in schedule. “Clinical worsening” and “suddenly discontinuing” are not defined in the dictionary, leaving them to interpretation. Expect inconsistency in decision-making.

Step 1: Confirm applicable standard of proof. Determine whether the veteran has relevant service as defined. If the veteran has operational service, this SoP does not apply; use the reasonable hypothesis SoP. Only proceed under this SoP if the service is eligible war service (non-operational), defence service (non-hazardous/non-British nuclear test), or peacetime service under MRCA.

Step 2: Establish the diagnosis of rheumatoid arthritis exactly as defined. Obtain a rheumatologist’s report that confirms symmetric inflammatory peripheral polyarthritis persisting at least six weeks, with inflammatory synovitis, and excludes juvenile RA, psoriatic arthritis, acute viral polyarthritis, SLE, and polyarticular gout. The report should explicitly state that the condition meets the definition in subsection 7(2). If the diagnosis is uncertain or involves a non-rheumatoid condition, the claim will fail. The ICD code is irrelevant; use the clinical definition.

Step 4: Identify which factors might apply. Review the twelve factors. The most common are likely smoking (factor 1 or 6), obesity (factor 5 or 11), and periodontitis (factor 4 or 10). For each potential factor, collect evidence of the required dosage and timing.

For smoking: Obtain a sworn statement or interview documenting smoking history (brands, daily quantity, start and stop dates, any periods of cessation). Calculate pack-years: (cigarettes per day ÷ 20) × years of smoking. If the veteran used other tobacco products, calculate weight in kilograms and convert to pack-years. If smoking ceased, ensure that clinical onset occurred within ten years (for onset) or two years (for worsening). For silica dust: Obtain evidence of employment or service duties involving production, excavation, drilling, cutting, grinding, or use of materials containing crystalline silica. Document the number of hours spent in these activities. Keep logs, pay records, witness statements, or historical records. Calculate cumulative hours; must be at least 5,000 within the ten years before onset or worsening. Many veterans will struggle to provide this level of detail for service decades ago. Consider whether the work was continuous or intermittent.

For drugs (factors 3 and 8): Obtain medical records showing treatment with GM-CSF or interferon-α within eight weeks before clinical onset or worsening. This is rare; if it does not apply, move on.

For periodontitis: Obtain a dental report confirming periodontitis (not just gingivitis) and that it was present for at least three months before the relevant clinical event. The periodontitis diagnosis must itself meet the definition in its own SoP (via section 11). Request a copy of the Statement of Principles for periodontitis and ensure the claimant satisfies its factors.

For obesity: Document weight and height over time. The best evidence is medical records showing BMI calculations or raw weight/height data at multiple points over at least five years within the ten-year window before onset or worsening. If no contemporaneous records exist, the claimant will need to reconstruct weight history through sworn evidence and corroboration (e.g., clothing sizes, photographs, witness accounts). The decision-maker may be sceptical of retrospective BMI estimates.

For discontinuing DMARDs: Confirm continuous treatment for at least six months prior to cessation. The DMARD must be one listed in the definition. Obtain evidence of the cessation (e.g., medical notes, prescription records) and evidence that it was sudden, not a gradual taper. Show that clinical worsening (objective increase in disease activity) occurred within six months of cessation.

For inability to obtain appropriate clinical management: This is a fallback factor. Document any barriers the veteran faced in accessing rheumatology care related to their service (e.g., posting to a base without a specialist, denial of treatment, lack of diagnosis). The factor does not require proof of other factors; it is standalone. However, it is likely the hardest to prove because the claimant must show that the inability contributed to the onset or worsening, not just that care was unavailable.

Step 5: Link the factor to relevant service. The factor must have occurred as a result of, or during, the veteran’s service. For example, smoking may have started or increased due to service stress; silica exposure occurred during service duties; obesity developed due to service-related injury limiting exercise. Obtain documentary or witness evidence of this link. If the factor is unrelated to service (e.g., smoking started after discharge for non-service reasons), the claim will be rejected.

Step 6: Compile all evidence and submit a detailed claim with references to each section of the SoP. Use the exact language of the instrument. For example, state that “the veteran smoked at least five pack-years of cigarettes before the clinical onset of rheumatoid arthritis, and where smoking ceased, the clinical onset occurred within ten years of cessation, and the smoking is related to relevant service because [reason].” This makes it easier for the decision-maker to apply the SoP. Include copies of all source documents.

Step 7: If the claim is denied, identify the specific reason. Common denial reasons: factor not proven, temporal window not met, factor not related to service, definition of RA not satisfied. Address these in internal review or before the AAT. Consider expert evidence to clarify definitions. If the decision-maker misapplied the SoP, argue the point on the precise language.

For legal representatives: Be alert to the cross-referencing under section 11. If a factor like periodontitis is relied upon, you must concurrently prove the periodontitis under its own SoP. That may require additional evidence. Do not rely on a factor without checking its SoP. Also monitor any amendments to linked SoPs as they will automatically apply.

Compliance is not about satisfying a regulatory obligation but about navigating a rigid causation framework. The instrument leaves little room for medical judgment or sympathy; the evidence must fit the precise definitions. The more contemporaneous, documented evidence the claimant can provide, the higher the chance of success.
