If the launch is to be conducted from a facility, the application must include the following information: (a) the safety record of the facility; (b) if the safety record includes anomalies or failures, the measures taken to reduce the risk of future anomalies or failures; (c) the licensing or accreditation the facility has received; (d) the standards to which the facility was designed and built and under which the facility operates.
