13 Application for supervision order (1) The Secretary may apply to the court referred to in section 12 for a supervision order in respect of a person who is an eligible offender at the time when the application is commenced. (b) at least one assessment report in respect of the eligible offender; and (c) if the Secretary intends to request the imposition of an intensive treatment and supervision condition, a treatment and supervision plan. (3) As soon as practicable after an application under subsection (1) is commenced, the Secretary must serve on the eligible offender— (b) a copy of each document filed with the application; and (iii) the nature of a supervision order. (4) An application under subsection (1) must be discontinued if the offender ceases to be an eligible offender by reason of section 8(4).
