14 Application fee for renewal of rehabilitation program provider approval

For the purposes of subsection 34K(2) of the Act, the prescribed fee for processing an application for renewal of a person as a rehabilitation program provider is as follows: (a) if the person provides a rehabilitation program in one State or Territory—$1,000; (b) if the person provides a rehabilitation program in 2 or 3 States or Territories—$3,000; (c) if the person provides a rehabilitation program in 4 or more States or Territories—$5,000.
