17 Information to be provided in or with approved application forms The Secretary may require the following information to be included in or to accompany an approved application form under regulation 16(1)— (a) the vehicle particulars; (b) the personal particulars or corporation particulars of the applicant; (c) the customer number of the applicant, if known; (d) evidence verifying the information in the approved application form; (e) any additional information the Secretary considers necessary to enable the Secretary to decide whether or not to grant the application.
