61O Form of approval
An approval to use race field information must include the following:
(a) the name and business address of the licensed wagering
operator;
(b) the date of issue of the approval;
(c) the end date of the approval;
(d) any condition imposed on the approval under section 61N;
(e) an identifying number for the approval.
