48 Report of birth A report of a birth in the form approved by CCOPMM must be submitted to CCOPMM within the prescribed period by— (a) if the birth occurs in a health service, the proprietor of the health service; or (b) if the birth does not occur in a health service— (i) the midwife who is in attendance upon the mother at the time of the birth; or (ii) if subparagraph (i) does not apply, the registered medical practitioner who is in attendance upon the mother at the time of the birth; or (c) if paragraphs (a) and (b) do not apply— (i) if the mother or the child is admitted to a health service because of the birth of the child, the proprietor of the health service; or (ii) in any other case, the registered medical practitioner who undertakes the care and treatment of the mother or the child because of the birth of the child.
