56H Registration—certificate
A registration certificate must include the following information:
(a) the name of the registered person;
(b) the nature of the registered activity (and whether it is
location-specific);
(c) the location for which the activity is registered, including the
address of the premises (or each premises), and the precise
location on the premises, where the activity is authorised to be
carried on;
(d) the period for which registration is granted;
(e) the name and address of the owner of the registered premises (or
each registered premises);
(f) the name and address of the manager of the registered premises
(or each registered premises);
(g) the name and address of the owner of the equipment associated
with the carrying on of the registered activity;
(h) the name and address of the manager of the equipment
associated with the carrying on of the registered activity.
