105 Procedure register
(1) A facility must keep a procedure register.
(2) The procedure register must contain details of every surgical procedure carried out at the facility, including the following—
(a) the patient’s full name and medical record number,
(b) the name of the procedure,
(c) the names of surgeons, surgical assistants, anaesthetists and scrub nurses involved in the procedure,
(d) the anaesthetic administered,
(e) the date, time and place of the procedure.
