#### 11 Form of licence 11 Form of licence > Without limiting the particulars that may be included in a licence, a licence is to specify— > > > (a) the person to whom it is issued, and > > > (b) the address of the private health facility for which it is issued, and > > > (c) the class of facility in respect of which it is issued, and > > > (d) the maximum number of patients who can be accommodated at any one time in each ward of the facility, and > > > (e) any conditions to which the licence is subject (other than the conditions referred to in section 12 (2)).
