sch.11-sec.74 Receipt for seized property— Act , s 622

For section 622 (5) of the Act , all of the following information is required in relation to a receipt, given or left by a police officer or protective services officer, for a thing seized by the officer under the Act — the date and time the thing was seized; if the thing was seized from a person—the name, address and telephone number of the person, if known; if the thing was seized from an occupied place—the name, address and telephone number of the occupier of the place, if known; a description of the thing; the officer’s name, rank, registered number, station and telephone number; where the thing will be taken, if known; the date the receipt is issued.

• (a) the date and time the thing was seized;
• (b) if the thing was seized from a person—the name, address and telephone number of the person, if known;
• (c) if the thing was seized from an occupied place—the name, address and telephone number of the occupier of the place, if known;
• (d) a description of the thing;
• (e) the officer’s name, rank, registered number, station and telephone number;
• (f) where the thing will be taken, if known;
• (g) the date the receipt is issued.
