sec.59 Application by licence holder
The holder of a pharmacy business licence may apply to the council to change any of the following stated on the licence— the business name for the pharmacy business to which the licence relates; a condition of the licence; if the name of a holder of the licence, a director of a holder of the licence that is a corporation, or a person who holds a material interest in the pharmacy business to which the licence relates, has changed—the name of the holder, director or person. a holder of a pharmacy business licence that is a corporation changes its name a person who holds a material interest in a pharmacy business changes the person’s surname The application must be— in the approved form; and accompanied by the application fee prescribed by regulation. - (a) the business name for the pharmacy business to which the licence relates;
- (b) a condition of the licence;
- (c) if the name of a holder of the licence, a director of a holder of the licence that is a corporation, or a person who holds a material interest in the pharmacy business to which the licence relates, has changed—the name of the holder, director or person.
