sec.25 Requirements for application

An application for a pharmacy business licence must— be in the approved form; and state the following— the address of the premises at which the pharmacy business to which the application relates is, or is to be, carried on; if the applicant is a corporation—the name of each director and shareholder of the applicant; if the applicant is, or is to be, an owner of the business as trustee of a trust—the name of each beneficiary of the trust; if a person holds shares in the applicant as trustee of a trust—the name of each beneficiary of the trust; the name of each person who, to the best of the applicant’s knowledge, holds a material interest in the business mentioned in section 13 (1) (c) ; and be accompanied by the application fee prescribed by regulation. The application must not state more than 1 address for premises for subsection (1) (b) (i) . s 25 amd 2025 No. 21 s 7 (sec.25-ssec.1) An application for a pharmacy business licence must— be in the approved form; and state the following— the address of the premises at which the pharmacy business to which the application relates is, or is to be, carried on; if the applicant is a corporation—the name of each director and shareholder of the applicant; if the applicant is, or is to be, an owner of the business as trustee of a trust—the name of each beneficiary of the trust; if a person holds shares in the applicant as trustee of a trust—the name of each beneficiary of the trust; the name of each person who, to the best of the applicant’s knowledge, holds a material interest in the business mentioned in section 13 (1) (c) ; and be accompanied by the application fee prescribed by regulation. (sec.25-ssec.2) The application must not state more than 1 address for premises for subsection (1) (b) (i) .

• (a) be in the approved form; and
• (b) state the following— (i) the address of the premises at which the pharmacy business to which the application relates is, or is to be, carried on; (ii) if the applicant is a corporation—the name of each director and shareholder of the applicant; (iii) if the applicant is, or is to be, an owner of the business as trustee of a trust—the name of each beneficiary of the trust; (iv) if a person holds shares in the applicant as trustee of a trust—the name of each beneficiary of the trust; (v) the name of each person who, to the best of the applicant’s knowledge, holds a material interest in the business mentioned in section 13 (1) (c) ; and
• (i) the address of the premises at which the pharmacy business to which the application relates is, or is to be, carried on;
• (ii) if the applicant is a corporation—the name of each director and shareholder of the applicant;
• (iii) if the applicant is, or is to be, an owner of the business as trustee of a trust—the name of each beneficiary of the trust;
