#### 271 How to apply (1) The application must: (a) be made in accordance with the approved form; and (b) be made in the approved manner; and (c) include details of the activities that the applicant proposes to carry out; and (d) be accompanied by a map that shows the proposed location of the activities; and (e) specify an address for service of notices under this Act and the regulations. > Note: For paragraphs (a) and (b) see section 41. (2) The applicant may include in the application any other information that the applicant thinks is relevant. (3) The application must be lodged with the Designated Authority.
