37 Form of applications
An application under Division 2, 3 or 6 must: (a) be made in accordance with a form approved, in writing, by the CEO; and (b) be accompanied by the application fee (if any) prescribed by the rules; and (c) contain the information that the form requires; and (d) set out the reasons for the application; and (e) satisfy any other requirement prescribed by the rules for the purposes of this paragraph.
