sec.37 Signed statement of adult who was subject to Ailan Kastom child rearing practice
A signed statement of an adult who was subject to Ailan Kastom child rearing practice must address the following matters— how the person was made aware that they were the subject of Ailan Kastom child rearing practice; what information the person was provided with about the Ailan Kastom child rearing practice; the nature of the relationship the person has with their cultural parents; how the person refers to the cultural parents the nature of any relationship the person has with their birth parents; how the person refers to the birth parents that the person consents to the making of inquiries of, and the exchange of information with, the person’s birth parents, cultural parents and informed person for the purpose of deciding whether to make a cultural recognition order; that the person gives their informed consent to the application.
