69 Form of licence A licence: (a) is to be in the approved form; and (b) is subject to the conditions that are determined by the CEO and specified in the licence; and (c) is valid only in respect of the electroconvulsive therapy specified in the licence; and (d) remains in force for the period, not longer than 3 years, specified in the licence.
