#### 125 Requirements with respect to application 125 Requirements with respect to application > An application for an extension order must be supported by documentation— > > > (a) that addresses each of the matters referred to in section 127(2) (to the extent relevant to the application), and > > > (b) that includes a report (prepared by a qualified psychiatrist, registered psychologist or registered medical practitioner)— > > > > > (i) that assesses the risk of the forensic patient causing serious harm to others, and > > > > > (ii) that addresses the need for ongoing management of the patient as a forensic patient and the reasons why the risk of the forensic patient causing serious harm to others cannot be adequately managed by other less restrictive means.
