102 Application for a certificate
(1) A person may apply to the Secretary for any of the following certificates—
(a) a certificate of release;
(b) a certificate of staged payment approval;
(c) a certificate of partial release;
(d) a certificate of no liability;
(e) a levy liability certificate.
(2) An application must be—
(a) in the form approved by the Secretary; and
(b) accompanied by the information and documents that the Secretary specifies.
(3) The Secretary may require the person to verify, by a statutory declaration signed by the person, any information—
(a) in the form or accompanying the application; or
(b) in a document accompanying the application.
