122 Noting changes to prescriptions on oral direction of prescriber—Act, s 27 (2) (b) (ii) The following must be noted, in writing, on the prescription: (a) the name of the prescriber giving the oral direction to change the (b) the change to the prescription; (c) the date the oral direction is given; (d) the pharmacist’s signature. Note The notation must be made as soon as possible (see Legislation Act, s 151B).
