This Act has been repealed and is no longer in force. It is retained for historical reference.
What it does: Prescribes the standard forms and certificates used under the Navigation Act for ship surveys and certification — e.g. Certificate of Survey, Record of Equipment, Exemption Certificate, alteration notices, incident alert/report and Passenger Certificate — and the endorsements and periodic-survey entries that must appear on them.
Who it affects and why it matters: Ship owners/operators, masters, classification societies and AMSA officials must complete, issue and retain these documents to show legal survey status, record safety equipment and meet incident‑reporting and exemption procedures.
