15 Declaration form

A declaration form must require the person voting to state the following information—

(a) the person’s name,

(b) that the person is an individual at least 18 years of age,

(c) that the person is a ratepayer, or the nominee of a ratepayer, of a holding in the region consisting wholly or partly of rateable land,

(d) if the person is voting in the person’s capacity as a ratepayer—that the person has not yet voted in the election in the person’s capacity as a ratepayer.
