191 Application process (1) The application must be in the approved form and lodged with the (2) The application must include the following: (a) the name and contact information of the applicant and any representative of the applicant; (b) a detailed explanation of the restriction being sought for the place; (c) a detailed description of the proposed restricted premises; (d) the reasons for the proposed restriction; (e) if the applicant is not the occupier of the place: (i) the name and contact information of the owner; and (ii) a statement of whether the owner was informed of the application. (3) The Director may request additional information from the applicant or applicant's representative. (4) The Director may reject an application if satisfied the application is frivolous or vexatious.
