64 Permit—surrender (1) A permit-holder may surrender the permit by giving written notice (a surrender notice) of the surrender to the commissioner. (2) The surrender notice must be accompanied by— (a) the permit; or (b) if the permit has been lost, stolen or destroyed—a statement verifying that the permit has been lost, stolen or destroyed.
