80L Application for review of determination (1) For section 352(2)(b) of the Act: (a) an application for a review of a determination of a cost assessor must be in the approved form and be accompanied by the following: (i) an affidavit that notice of the application has been given to the other parties; (ii) a copy of all the costs assessor's certificates of determination relating to the assessment that is the subject of the application; (iii) a copy of the costs assessor's statement of reasons for the determination; and (b) the applicant must give a copy of the application to each of the other parties. (2) For section 352(2)(c) of the Act, the fee is 275 revenue units.
